Who is eligible?

• Rockdale County residents only

• At least 18 years of age or older

• Must have diagnosis of hypertension, diabetes, COPD/Asthma, or heart disease

• Low or no income

• Uninsured and no access to health insurance

How Do I get Scheduled?

• Referral from hospital can be made and a clinic staff member will call patient to follow up

• Self-referrals are accepted; new patients should call 678-374-5134 on Monday between 10:00 am – 12:00 pm

• Eligibility Specialist will conduct phone screening for qualification

• No urgent or emergency appointments available
• Eligibility Specialist will also let patient know what to bring to their initial visit

What time will my appointment be scheduled?

• Medical clinic is open on Tuesday evenings and Friday during the daytime hours
• No walk-ins accepted

Dental Services

Who is eligible?

• Rockdale County residents only

• At least 18 years of age or older

• Low or no income
• Uninsured and no access to dental insurance

What services are offered?

• Exams, x-rays, cleanings, fillings, and extractions only

• No dentures or partials
• No crowns, root canals, etc.

How do I get scheduled?

• New patients should call 678-374-5134 on Mondays between 10:00 AM – 12:00 PM

• Eligibility Specialist will conduct phone screening for qualification

• No urgent or emergency appointments available
• Eligibility Specialist will also let patient know what documentation to bring to their initial visit

What time will my appointment be scheduled?

• Dental clinic is open two days/week. Current dental days are Monday and Thursday
• No walk-ins accepted
